To help us learn as much as possible about your exotic pet and their medical history, please obtain any previous records that you might have. Let us know if you prefer, and we can obtain the records for you.
Please fill out the following form before your scheduled appointment so we can properly prepare your exotic pet’s medical record in advance of your visit. If you have not made an appointment yet, please call the office at 704-932-8111 to schedule an appointment.
